Too big to be a W-Series. This is actually a T-Series, which is bigger. Here's a newer model.

1986-90 GMC Forward, T-Series didn't come until 1997-09.

As they were built in Japan by Isuzu, it should be only "Made for: USA".

They made them in General Motors plant in Janesville, Wisconxin

Not all of them. Some were still imported from Japan.
The only model with this bumper was the heaviest weight class. W7.
