 I'm not sure where it was filmed but the movie takes place in Germany. Cars have German plates, the actors have German names etc...

 The director and most of the cast and crew listed on the IMDB are Italian, so it was probably filmed in Italy.

 So maybe this is the reason for the faked German plate on the faked (for Germany) Citroen-Ambulance, that the holder was for Italy. Otherwise I cannot see, that the Mercedes has the special Italian back bumper and neither (at the BMW, too) the side indicators.
